Child Health Report
Required State Form
A Child Health Report is the responsibility of the legal parent/guardian of the child, to be completed as required per PA DHS & OCDEL. Children under the age of 2 need to have this form updated every six months. Children age 2 and older need to have this form updated every one year.
Emergency Contact
Required State Form
An Emergency Contact is the responsibility of the legal parent/guardian of the child, to be completed upon registration and should be updated every six months, or sooner should personal information change for a family.
Medication Log
Required State Form (when applicable)
A Medication Log is the responsibility of the legal parent/guardian of the child, to be completed any time an over the counter or prescription medication will be sent in and needed to be administered by JJCC LLC Staff, to your child.
Over the counter medications only require this form to be completed.
Prescription medications will require this form to be completed and a prescribing Physicians or Doctors medical note affirming the prescription information, instructions and details. All original RX labels and storage must be in tact.
Medications of any kind may not be expired or missing an expiration date. A medication is considered as anything with an "active ingredient" or a "keep out of reach of children" or similar warning labels.
Child Daycare Agreement

A Child Daycare Agreement is the responsibility of the legal parent/guardian of the child, to be completed upon registration and should be updated every six months, or sooner should any personal information change or if the child's plans in attendance should need change.
